1. Information proving adequate sickness cover is proven by consulting the databases of the National Health Service (SNS).

2. Where it is not possible to access the information as provided in the preceding paragraph, it shall be proven as follows:

a) In the case of information on registration with the SNS, by a declaration issued by an SNS establishment or service, in compliance with Article 51(8);
a) Tratando-se de informação relativa à inscrição no SNS, através de declaração emitida por estabelecimento ou serviço do SNS, com respeito pelo disposto no n.º 8 do artigo 51.º;
b) In the case of information on health insurance taken out with a national insurance company, by a declaration issued by that company, in compliance with Article 51(8);
b) Tratando-se de informação relativa a seguro de saúde contratado junto de companhia de seguros nacional, através de declaração emitida pela mesma, com respeito pelo disposto no n.º 8 do artigo 51.º;
c) In the case of information on health insurance taken out with a foreign insurance company, by a declaration issued by that company, in compliance with Article 51(9).
c) Tratando-se de informação relativa a seguro de saúde contratado junto de companhia de seguros estrangeira, através de declaração emitida pela mesma, com respeito pelo disposto no n.º 9 do artigo 51.º
3. Information proving the continuity of medical treatment and the guarantee of hospital admission or outpatient treatment in an official or officially recognised health establishment, or that the applicant is on a waiting list or in the integrated management system for patients awaiting surgery, is proven by consulting the SNS databases.
3 - A informação comprovativa da continuidade do tratamento médico e da garantia de internamento hospitalar ou de tratamento em regime ambulatório em estabelecimento de saúde oficial ou oficialmente reconhecido ou de que o requerente se encontra em lista de inscritos ou no sistema integrado de gestão de inscritos para cirurgia é comprovada através de consulta às bases do SNS.
4. Where it is not possible to access the information as provided in the preceding paragraph, it shall be proven, as applicable, by a declaration issued by an SNS establishment or service or by the official or officially recognised health establishment, in compliance with Article 51(8).
4 - Quando não seja possível aceder à informação nos termos previstos no número anterior, deve a mesma ser comprovada, consoante o caso, através de declaração emitida por estabelecimento ou serviço do SNS ou pelo estabelecimento de saúde oficial ou oficialmente reconhecido, com respeito pelo disposto no n.º 8 do artigo 51.º
