The visa application, submitted on the appropriate form, shall contain the following particulars:
Do pedido de visto, apresentado em formulário próprio, devem constar os seguintes elementos:
a) Full identification of the applicant and, where the applicant holds a family passport or a collective passport, of the spouse, dependants or group members named in it who wish to benefit from the visa, where applicable;
a) A identificação completa do requerente e, caso seja titular de passaporte familiar ou de passaporte colectivo, do cônjuge, dos dependentes ou dos elementos do grupo que neles se encontram mencionados que pretendam beneficiar do visto, quando aplicável;
b) The type, number, date and place of issue and validity of the travel document, and identification of the authority that issued it;
b) O tipo, número, data e local de emissão e validade do documento de viagem e a identificação da autoridade que o emitiu;
e) The name of the host person or host entity and the name of the contact person at the host entity, where applicable;
e) Nome da pessoa ou da empresa de acolhimento e nome da pessoa a contactar na empresa de acolhimento, quando aplicável;
f) The intended place of accommodation, where applicable.
f) Local previsto de alojamento, quando aplicável.
