Without prejudice to Article 42-A, an application for the residence permit provided for in Article 123-A of Law No. 23/2007 of 4 July, as amended, shall be accompanied by documents containing the following information:
b) Information proving an employment relationship or the pursuit of a self-employed professional activity;

c) Information proving registration with social security;
c) Informação comprovativa da inscrição na segurança social;
d) Information proving the grant of residence by the country of provenance;
d) Informação comprovativa da concessão de residência do país de proveniência;
e) Criminal record information from the country of previous residence;
e) Informação sobre antecedentes criminais do país da anterior residência;
f) Criminal record information in Portugal, obtained by the Agency for Integration, Migration and Asylum (AIMA).
